Чувствительность артериального барорефлекса является важным маркером, отражающим состояние автономной регуляции кровообращения. Настоящая работа посвящена новому подходу в оценке спонтанного артериального барорефлекса, позволяющему улучшить качество анализа данных, получаемых при неинвазивной непрерывной регистрации артериального давления и ритмограммы. Предложенный метод оценки совместной плотности вероятности артериального давления и R-R интервалов позволяет повысить эффективность использования фрагментов записи за счет включения в анализ большего числа отчетов, отличается лучшей воспроизводимостью и, вероятно, чувствительностью, по сравнению с традиционными методиками. Кроме того, предложенный метод позволяет оценить ряд дополнительных показателей, характеризующих артериальный барорефлекс. Все это существенно расширяет сферу использования характеристик спонтанного барорефлекса за счет возможности применения новой методики на более коротких записях, в том числе при ряде переходных состояний, упрощая расчеты и сокращая время записи гемодинамических параметров.</p></abstract><trans-abstract xml:lang="en"><p>Autonomic dysfunction is associated with negative prognosis in cardiovascular patiens and several other pathologies. Arterial baroreflex sensitivity is an important marker of autonomic regulation. Present work is devoted to the new approach to spontaneous baroreflex estimation, which permit to improve quality of the continious blood pressure and ECG monitoring analysis. The method of combined estimation of probability density of blood pressure and R-R intervals is proposed in this article, which improves efficacy of use of date records due to the increased number of included beats in calculation. This method is appeared to have better reproducibility and, probably, sensitivity for autonomic dysfunction diagnosis compared to traditional approaches. Moreover, offered method allow to measure additional arterial barorexlex characteristics.
