Оценить состояние стенки общих сонных артерий у больных артериальной гипертензией с различными вариантами ремоделирования левого желудочка (ЛЖ). Материалы и методы. Обследовано 102 больных 30-65 лет с эссенциальной артериальной гипертензией. Средняя продолжительность артериальной гипертензии составила 7,1 ± 1,2 года. Проводилось ультразвуковое исследование сердца и общих сонных артерий. Были сформированы подгруппы в зависимости от варианта геометрического ремоделирования ЛЖ по рекомендациям A. Canau и соавт. (1992). Оценивались структурные изменения стенки общих сонных артерий и параметры центральной гемодинамики. Результаты. В подгруппе пациентов с концентрической гипертрофией ЛЖ определялись высокие значения индекса жесткости (6,81 ± 1,1 усл. ед.) и толщины комплекса интима-медиа (1,0 ± 0,18 мм) сосудистой стенки, достоверно отличающиеся от подгрупп с нормальным вариантом ЛЖ и эксцентрической гипертрофией ЛЖ (ЭГЛЖ) (р &lt; 0,05). Больные с ЭГЛЖ имели больший внутренний диаметр сонных артерий, составивший 6,42 ± 0,2 мм, по сравнению с другими подгруппами (р &lt; 0,05). Выводы. При эссенциальной артериальной гипертензии наблюдаются различия структурных особенностей стенки эластических артерий в зависимости от варианта ремоделирования левого желудочка.</p></abstract><trans-abstract xml:lang="en"><p>Objective: to estimate common carotid arteries wall state in hypertensive patients with different types of left ventricle (LV) remodeling.
Materials and methods: 102 patients 30-65 years old with essential arterial hypertension were examined. The average arterial hypertension length was 7,1 ± 1,2 years. Ultrasound examination of heart and common carotid arteries was performed. The subgroups were formed depending on the type of geometric left ventricle remodeling recommended by A. Canau et al. (1992). Structural changes of common carotid arteries walls and central hemodynamics parameters were estimated. Results: patients with concentric left ventricle hypertrophy demonstrated high arterial stiffness index (6,81 ± 1,1 units) and thickness of arterial intima-media complex (1,0 ± 0,18 mm), significantly differing from subgroups with normal type of LV and eccentric hypertrophy of LV (EHLV) (p &lt; 0,05). Patients with EHLV have bigger internal diameter of carotid arteries (6,42 ± 0,2 mm) as compared to other subgroups (p &lt; 0,05).
