Кардиоренальный континуум беременных женщин с артериальной гипертензией

Проведен анализ факторов риска (ФР), начальных стадий, прогрессирования и конечной стадии сердечно-сосудистых заболеваний (ССЗ) и хронической болезни почек (ХБП) в динамике гестации у 159 женщин с гипертензивными нарушениями в сравнении сданными 32 здоровых беременных. Отдельно исследованы факторы кардиоренального континуума у женщин с различными вариантами гестационной артериальной гипертензии (ГАГ): у 13 - с изолированной клинической АГ (ИКАГ), 11 - с изолированной амбулаторной АГ (ИААГ) и 18 - с АГ, установленной всеми тремя методами (клиническое, амбулаторное и домашнее измерение артериального давления (АД)). По мере увеличения числа методов измерения АД, фиксирующих его повышение у беременных с ГАГ (клиническое АД (AIL) при ИКАГ, суточное мониторирование (СМАД) и самоконтроль АД (СКАД) при ИААГ, АД, СМАД и СКАД при «обычной» АГ), росло количество ФР (возраст, семейный анамнез ССЗ и гестоза, курение, гиподинамия и гормональная контрацепция в анамнезе, уровень АД, абдоминальное ожирение, дислипидемия, тревожно-депрессивный синдром, оксидативный стресс, нарушение гликемии натощак, метаболический синдром), а также частота субклинического поражения органов (дестабилизация клеточных мембран, гипертрофия левого желудочка (ГЛЖ). уголщеиие интимы-медии, небольшое повышение уровня сывороточного креатинина, гиперурикемия, эндотелиальная дисфункция, альбуминурия, снижение скорости клубочковой фильтрации). Частота и выраженность ФР и признаков поражения органов-мишеней, а также общий риск сердечно-сосудистых осложнений у здоровых беременных и в подгруппах женщин с ГАГ выстроились в возрастающую последовательность «здоровые - ИКАГ - ИААГ -АГ», то есть в континуум, где каждое звено мало чем отличается от соседнего, но различия между крайними звеньями становятся высокодостоверными. Оценка кардиоренального состояния у всей когорты беременных также позволила сформировать континуум «здоровые беременные - ГАГ- гипертоническая болезнь (ГБ) I стадии - ГБ II стадии - гестоз - сочетанный с ГБ гестоз», по мере удаления от начального звена которого возрастает риск возникновения ССЗ и ХБП с потенциальной опасностью развития сердечно-сосудистых осложнений и хронической почечной недостаточности.

Risk factors (RF), initial stages, progression, and final stage of both cardiovascular disease (CVD) and chronic kidney disease (CKD) were analyzed in a cohort of 159 pregnant women with hypertensive disorders versus a cohort of 32 healthy pregnant controls. Cardiorenal continuum factors were separately investigated in patients with different gestational hypertension (HT) depending on the diagnostic method: 13 with isolated clinic HT (1CHT), 11 - isolated ambulatory HT (IAHT), and 18 - HT found by all three blood pressure (BP) measurement methods (clinic, ambulatory, and home BP). The number of RF (age, family history of CVD and pre-eclampsia, pre-pregnancy history of smoking, lack of physical exercise, and oral contraception, BP levels, abdominal obesity, dyslipidaemia, anxiety and depression, oxidative stress, altered fasting plasma glucose, metabolic syndrome), signs of subclinical organ damage (cell membrane destabilization, left ventricular hypertrophy, intima-media thickening, slight increase in serum creatinine, hyperuricaemia, endothelial dysfunction, albuminuria, low glomerular filtration rate), and total cardiovascular risk progressively increased from the condition of being normotensive at the time of office, home, and 24-hour measurements to the condition of being found hypertensive by one, two and all three BP measurement methods, forming the continuum «healthy pregnant women - ICHT - IAHT - gestational HT». Assessment of cardiorenal state in all pregnant women allowed to compose the cline «healthy pregnants- gestational HT- stage I essential HT - stage II essential HT - pre-eclampsia - essential HT with superimposed pre-eclampsia» with the growing risk of CVD and CKD and potential danger of cardiovascular events and chronic renal failure.
