Агонисты имидазолиновых рецепторов, в частности, моксонидин, наряду с антигипертензивным действием, могут приводить к повышению чувствительности периферических тканей к действию инсулина, снижению уровня лептина, улучшению показателей липидного обмена. Вместе с тем эффективность, переносимость и плейотропные эффекты моксонидина у женщин с гипертонической болезнью (ГБ) на фоне метаболического синдрома (МС) в постменопаузе изучены недостаточно, что и послужило целью настоящего исследования. Материалы и методы. Обследовано 30 пациенток с ГБ и МС, в постменопаузе, их средний возраст составил 52,5 ± 0,4 года. Терапия моксонидином - 12 недель. Определяли следующие показатели: окружность талии (ОТ), окружность бедер (ОБ), соотношение ОТ/ОБ, индекс массы тела (ИМТ), уровень лептина, инсулина, глюкозы, липидный спектр сыворотки крови, индекс инсулинорезистентности (HOMA-IR) и индекс функциональной активности бета-клеток поджелудочной железы (НОМА-FB). Проводилось эхокардиографическое исследование и исследование толщины комплекса интима-медиа (КИМ) общих сонных артерий (ОСА). Результаты. Помимо отчетливого антигипертензивного эффекта, при применении моксонидина отмечены уменьшение массы тела и ИМТ, повышение холестерина липопротеидов высокой плотности и индекса НОМА-FB, уменьшились диаметр и объем левого предсердия, а также улучшилась диастолическая функция левого желудочка. Выводы. Моксонидин - эффективный антигипертензивный препарат для лечения женщин с ГБ на фоне МС в постменопаузе. На фоне лечения моксонидином зарегистрировано снижение массы тела и ИМТ, улучшение показателей углеводного и липидного обмена, улучшение показателей, характеризующих диастолическую функцию левого желудочка и уменьшение объема и диаметра левого предсердия.</p></abstract><trans-abstract xml:lang="en"><p>Background. Imidazoline receptor agonist moxonidine besides its antihypertensive effect, can increase insulin sensitivity, reduce leptin level, improve lipid metabolism. However, the efficacy, tolerability and pleiotropic effects of moxonidine in postmenopausal women with essential hypertension (EH) and metabolic syndrome (MS) are not well established, and it was the objective of this study. Design and methods. 30 postmenopausal women with EH and MS (52,5 ± 0,4 years old) were included. Moxonidine was administered (400-600 mkg/day)  for 12 weeks. Waist circumference (WC), hip circumference (HC), WC/HC ratio, body mass index (BMI), insulin, leptin level, glucose, lipid profile of blood serum, index of insulin resistance (HOMA-IR) and index of functional activity of pancreatic beta cells (HOMA-FB), echocardiography, intima-media thickness (IMT) of common carotid arteries (CCA). Results. Moxonidine therapy was associated with blood pressure (BP) decrease, body weight and BMI decrease, high-density lipoprotein (HDL) cholesterol and HOMA-FB index increase. Left atrium diameter and volume decreased, left ventricular diastolic function improved. Conclusions.
