Материалы и методы. Обследовано 73 пациента. Оценивались уровни глюкозы, инсулина, тиреотропного гормона (ТТГ) натощак. Индекс инсулинорезистентности (ИР) на основании гомеостатической модели (HOMA-IR) вычисляли по формуле: (инсулин натощак × глюкоза натощак)/22,5. Результаты. Уровень ТТГ был выше у пациентов с ожирением и избыточной массой тела (3,05 ± 1,14 2,73 ± 0,26 IU/I соответственно) по сравнению с лицами с нормальной массой тела. ИР был максимальным у пациентов с ожирением (4,67 ± 1,12). HOMA-IR оказался значимо ниже у лиц с нормальной и избыточной массой тела (1,30 ± 0,26 и 1,52 ± 0,74 соответственно, p &lt; 0,002 и 0,05). Выводы. Полученные данные свидетельствуют о наличии тесной взаимосвязи между ожирением, сахарным диабетом тип 2 (СД тип 2) и состоянием субклинического гипотиреоза у больных ишемической болезнью сердца (ИБС). Учитывая, что нарушение функциональной активности щитовидной железы вносит определенный вклад в развитие и прогрессирование атеросклероза, влияя на ключевые этапы катаболизма атерогенных липидов, дефицит тиреоидных гормонов потенцирует процессы атерогенеза. Гиперинсулинемия также способствует развитию дислипидемии. Вероятно, сочетание гиперинсулинемии и гипотиреоза у больных ИБС, особенно на фоне ожирения и СД тип 2, является крайне неблагоприятным фактором. В связи с этим активное выявление даже минимальной дисфункции щитовидной железы у таких больных представляется чрезвычайно важным.</p></abstract><trans-abstract xml:lang="en"><p>Objective. Thyroid disease is associated with atherosclerosis and cardiovascular disease. Little is known about the relationship between insulin resistance and thyroid function. The study addresses the relationship between thyroid function, body mass index (BMI) and homeostasis model assessment index for insulin resistance (HOMA-IR) in patients with heart ischemic disease. Design and methods. 73 patients were included in our study. Assessed parameters included age, gender and BMI. Fasting blood samples were taken for measuring of thyrotrophic hormone (TSH) level, glucose and insulin concentration. HOMA-IR was calculated as fasting insulin (mU/l) times fasting glucose (mmol/l) divided by 22,5. Results. TSH level was higher in patients with obesity (3,05±1,14 IU/l) and overweight (2,73 ± 0,26 IU/I) than in normal weight subjects, IR was the highest in the obese patients (4,67 ± 1,12). HOMA-IR was statistically lower in group I and II: 1,30 ± 0,26 and 1,52 ± 0,74 respectively (p &lt; 0,002 and 0,05). Conclusion.
