Оксид азота, синтезируемый эндотелиальной NO-синтетазой, играет важную роль в регуляции функции эндотелия и контроле артериального давления, частоты сердечных сокращений. Цель исследования - изучить связь полиморфизма интрона 4a4b гена эндотелиальной NO-синтетазы с уровнем артериального давления, частотой сердечных сокращений у пациентов с тиреотоксикозом болезни Грейвса. Материалы и методы. Мы определили полиморфизм гена eNOS 4a4b с помощью полимеразной цепной реакции у 164 пациентов с клиническим тиреотоксикозом и без сопутствующей соматической патологии. Всех пациентов мы разделили на три группы, сопоставимых по полу, возрасту, уровню тиреоидных гормонов: группа 1 состояла из пациентов с полиморфизмом eNOS 4b4b (n = 111), группа 2 - из пациентов с eNOS 4a4b полиморфизмом (n = 46), группа 3 - из пациентов с полиморфизмом eNOS 4a4a (n = 7). Мы определили уровень артериального давления и частоту сердечных сокращений до и после терапии β-адреноблокаторами и ингибиторами ангиотензин-превращающего фермента в течение трех недель. Результаты. 4a4b полиморфизм гена эндотелиальной NO-синтетазы коррелирует с уровнем артериального давления, частотой сердечных сокращений. До лечения артериальное давление, частота сердечных сокращений были достоверно выше в группе 1, чем у пациентов третьей группы. После терапии артериальное давление, частота сердечных сокращений значимо снизились в группе 3 по сравнению с данными показателями у больных первой и второй групп. Выводы. Наличие аллеля b эндотелиальной NO-синтетазы предрасполагает к развитию дисфункции эндотелия сосудов.</p></abstract><trans-abstract xml:lang="en"><p>Background. Nitric oxide synthesized by endothelial nitric oxide synthase plays an important role in endothelial function regulation as well as in blood pressure and heart rate control. Objective. To assess the association between eNOS gene polymorphism intron 4a/4b and hypertension development, tachycardia in patients with clinical thyrotoxicosis. Design and methods. The eNOS gene polymorphisms were examined by polymerase chain reaction. We compared eNOS gene polymorphism 4a4b in 164 patients with clinical thyrotoxicosis. All patients we divided into three groups matching by sex, age, level thyroid hormones: group 1 included patients with eNOS 4b4b polymorphism (n = 111), group 2 - patients with eNOS 4a4b polymorphism (n = 46), group 3 - patients with eNOS 4a4a polymorphism (n = 7). Blood pressure and heart rate were assessed before and after 3-week treatment with β-blocker and angiotensin converting enzyme inhibitors. Result. The eNOS 4a4b polymorphism gene correlates with the blood pressure level and heart rate. Before treatment blood pressure and heart rate were significantly higher in group 1 than in group 3. After treatment blood pressure and heart rate were significantly lower in group 3 in comparison with groups 1 and 2. Conclusion.
