в зависимости от ремоделирования сосудов и вариабельности ритма сердца (ВРС) у больных гипертонической болезнью (ГБ). Материал и методы. Изучали динамику структурно-функциональных показателей сердца и плечевой
артерии (ПА), суточного профиля артериального давления (АД) и ВРС при терапии индапамидом ретард (Арифон
ретард, Лаборатории Сервье) в течение шести месяцев. С этой целью больные были разделены на 2 группы:
1-ю группу составили 37 больных без признаков ремоделирования ПА, 2-ю группу - 48 больных со структурно-
функциональными изменениями ПА. Больным проводили суточное мониторирование АД и электрокардиографическое, эхокардиографическое исследования и ультразвуковое импульсное исследование ПА. Результаты. Выявлено,
что антигипертензивный эффект индапамида ретард в сравниваемых группах достоверно не различается. При этом
во 2-й группе отмечен регресс ремоделирования ПА и увеличение эндотелий-зависимой вазодилатации, а в 1-й
группе выявлены высокодостоверные сдвиги показателей ВРС, свидетельствующие о подавлении симпатической
гиперактивности. Заключение. Показано, что при отсутствии ремоделирования сосудов кардиопротективный и
антигипертензивный эффекты индапамида ретард реализуются за счет модификации вегетативного дисбаланса, а
при наличии ремоделирования сосудов преобладает роль прямого сосудистого эффекта.</p></abstract><trans-abstract xml:lang="en"><p>Objective. To compare the antihypertensive and cardiovascular protection effects of Indapamide retard in response to
arterial remodeling and heart rate variability (HRV) in hypertensive patients. Design and methods. Structural and functional
indices of heart and brachial artery (BA) by ultrasound examination, circadian blood pressure (BP) rhythm and HRV by 24-hour
ECG and BP monitoring were assessed at baseline and after 6-month therapy with Indapamide retard. Patients were divided
into 2 groups: 1st group included 37 patients without arterial remodeling; 2nd group - 48 patients with arterial remodeling.
Results. Antihypertensive effect of Indapamide retard did not differ in groups. Changes of HRV indices were signifi cantly
higher in 1st group, but the regression of vessel remodeling and increasing of endothelium-dependent vasodilation were
signifi cant in 2nd group. Conclusion. This study shows that cardiac protection and antihypertensive effects of Indapamide
retard realize through autonomic modulation effect in absence of vascular remodeling, but direct vascular effect is observed
