Результаты. Установлено, что у пациентов с туловищным типом ожирения имеются более выраженные изменения липидного спектра крови, носящие атерогенный характер, повышен уровень неэстерифицированных жирных кислот и в большей степени снижена чувствительность тканей к инсулину, чем у больных с глютеофеморальным типом ожирения. При наличии туловищного (но не глютеофеморального) ожирения отклонения изученных показателей липидограммы у больных без диабета и с сахарным диабетом тип 2 существенно не различались. Лечение производными бигуанидов - метформином - больных туловищным ожирением, не страдающих сахарным диабетом тип 2, привело к снижению массы тела, уменьшению соотношения окружности талии к окружности бедер, снижению инсулинорезистентности и гиперинсулинемии, а также способствовало значительному улучшению показателей липидограммы. Вывод. У больных туловищным ожирением целесообразно рано начинать применение производных бигуанидов - на стадии, предшествующей развитию полного метаболического синдрома.</p></abstract><trans-abstract xml:lang="en"><p>Objective. To study lipid and carbohydrate metabolism in patients with different types and degree of obesity. Results. Patients with abdominal obesity have more severe atherogenic lipid abnormalities, higher levels of nonesterified fatty acids and decreased tissue insulin sensitivity than patients with gluteofemoral obesity. Lipid parameters did not differ in obese patients with type 2 diabetes and obese non-diabetic patients. Treatment with metformin resulted in weight loss, decrease of waist-to-hip ratio, insulin resistance and hyperinsulinemia, and improvement of lipid profile in non-diabetic patients with abdominal obesity. Conclusion.
