Проведено изучение артериальной ригидности (АР) с целью уточнения ее роли в развитии церебральных нарушений при артериальной гипертонии (АГ) и атеросклерозе. Материалы и методы. Обследовано 60 больных с дисциркуляторной энцефалопатией (ДЭ) либо с острыми нарушениями мозгового кровообращения (ОНМК) в анамнезе, составивших 1-ю группу, и 23 пациента с факторами сердечно-сосудистого риска, не имевших цереброваскулярных нарушений (ЦВЗ), сформировавших 2-ю группу. АР оценивали по данным суточного бифункционального мониторирования электрокардиограммы и артериального давления с помощью прикладной программы «Vasotens» (МнСДП-3, BPLab, «Петр Телегин», Россия) на основе математической обработки записей осциллограмм давления и электрокардиограммы. Результаты. Установлено, что показатели АР в обследованных группах не имели значимых различий, что связано с сопоставимостью групп по основным характеристикам, включая наличие АГ, ишемической болезни сердца, сахарного диабета, хронической сердечной недостаточности, а также атеросклеротического поражения брахиоцефальных артерий. Вместе с тем показатели АР были выше у больных с ДЭ по сравнению с пациентами, перенесшими ОНМК. При анализе состояния сосудистой стенки в зависимости от структурных изменений вещества мозга во всей обследованной когорте пациентов выявлена ассоциация повышенной АР с наличием очаговых, преимущественно множественных и/или диффузных церебральных изменений, а также с расширением ликворной системы мозга. Выводы. Таким образом, повышенная АР является значимым фактором церебрального повреждения при АГ и атеросклерозе. Причем первоначально это повреждение может протекать асимптомно, а по мере накопления патологических изменений проявляется диффузной симптоматикой.</p></abstract><trans-abstract xml:lang="en"><p>Objective. To assess arterial stiffness (AS) and to determine its influence on cerebral damage in patients with arterial hypertension (AH) and atherosclerosis. Design and methods. The 1st group included 60 patients with cerebrovascular disorders: 23 with chronic vascular encephalopathy (CVE) and 37 with post-stroke lesions. 2nd group consisted of 23 patients with cardiovascular risk factors without any cerebrovascular disturbances. AS was assessed by 24-hour bifunctional ECG and blood pressure monitoring (data were processed by the computer program «Vasotens», MnSDP-3, BPLab, «Pyotr Telegin», Russia). Results. In both groups AS parameters did not differ in the groups considering matched main vascular risk factors. But AS was higher in patients with CVE. There was a significant association between increased AS and focal cerebral (particularly multiple deep lacunar lesions) or diffuse (leucoaraiosis) lesions and subarachnoid space and ventricular system expansion visualized by brain magnetic resonance imaging. Conclusion.
