Лечение артериальной гипертензии должно основываться на принципах медикаментозной органопротекции. Изменения параметров внутрисосудистого кровотока на фоне приема ингибиторов ангиотензинпревращающего фермента неизвестны. Исследование данного вопроса послужило целью настоящего исследования. Материалы и методы. Обследовано 40 пациентов с гипертонической болезнью I–II стадии, средний возраст 48,1 ± 14,8 года. Терапия фозиноприлом — 27 недель. Изучали диаметр общей сонной артерии, толщину комплекса интима-медиа, коэффициент растяжимости, индекс жесткости, пиковые систолические скорости пристеночного и центрального слоя кровотока в общей сонной артерии и их соотношение — индекс замедления пристеночного кровотока (ИЗПК). Результаты. Помимо отчетливого гипотензивного эффекта, фозиноприл вызывал улучшение упруго-эластических свойств сосудистой стенки, уменьшал толщину интима-медиа и увеличивал ИЗПК. Вывод. Увеличение ИЗПК на фоне приема фозиноприла может свидетельствовать о снижении вероятности турбулентного течения в общих сонных артериях.</p></abstract><trans-abstract xml:lang="en"><p>Background. Organoprotective features should be taken into account when antihypertensive therapy is administered. The infl uence of angiotensin converting enzyme inhibitor (ACEi) on intravascular blood fl ow is unknown. Design and methods. A total of 40 patients with essential hypertension stage I–II (mean age 48,1 ± 14,8 years) were included. All of them got fosinopril therapy for 27 weeks. Examination included: assessment of the common carotid artery diameter, intima-media complex thickness, distensibility coeffi cient, stiffness index, peak systolic velocity of the wall and the central layer of the blood fl ow in the common carotid artery and their relationship — the index of wall blood fl ow slowdown (IWBFS). Results. In addition to a distinct antihypertensive effect, fosinopril induces improvement of elastic properties of the vascular wall, reduces intima-media thickness and increases IWBFS. Conclusion.
