Скрытая артериальная гипертензия (АГ) представляет собой серьезную угрозу для здоровья пациентов и часто не выявляется при клиническом измерении артериального давления (АД). В связи с этим необходима разработка чувствительного и экономически выгодного теста для выявления у пациентов скрытой АГ. Цель исследования - оценка использования прессорного ответа на добровольную задержку дыхания (тест задержки дыхания на 30 секунд) для выявления скрытой АГ у лиц молодого возраста. Материалы и методы. В исследовании проводилось наблюдение за ответом АД на тест с 30-секундной задержкой дыхания у 269 молодых лиц (18-36 лет), не имеющих сердечно-сосудистых заболеваний, сахарного диабета, каких-либо хронических заболеваний, в том числе дыхательной системы, и не занимающихся спортом профессионально. Результаты. Результаты теста задержки дыхания и 24-часового мониторинга АД согласовывались в 250 из 269 случаев (93 %). Важно, что ни у одного пациента с нормальным или высоким нормальным АД в покое и с отрицательным результатом теста задержки дыхания не выявлена скрытая АГ по результатам 24-часового мониторинга АД, что показывает прогностическую ценность отрицательного результата теста 100 %. Для лиц с нормальным или высоким нормальным АД в покое положительный тест задержки дыхания (53 пациента) выявил скрытую АГ у 34 пациентов, что обусловливает прогностическую ценность положительного результата 64 %. Выводы. Учитывая контролируемые условия проведения исследования, тест с задержкой дыхания позволяет эффективно исключать скрытую АГ у действительно здоровых лиц и точно выявлять лиц, которым впоследствии необходимо проводить исследования для диагностики гипертензии.</p></abstract><trans-abstract xml:lang="en"><p>Background. Latent arterial hypertension (LAH) is a serious health problem and is often underdiagnosed in routine examination. Thus, a sensitive and economical test to detect latent hypertension is needed. Objective. To assess the opportunity of pressure response to voluntary breath holding (30 second breath holding test) to detect masked arterial hypertension in young subjects. Design and methods. 269 young subjects (18-36 years old) with no clinical signs of cardiovascular disease, diabetes mellitus, any chronic diseases including pulmonary diseases, who did not do sports professionally, underwent 30 second breath holding test. Results. The results of breath holding and 24 hour monitoring tests coincided in 250 out of 269 cases (93 %). Latent hypertensions was diagnosed (by 24 hour blood pressure monitoring) in none of the subjects with normal or high normal resting blood pressure and negative breath holding test (negative predictive value - 100 %). For subjects with normal or high normal resting blood pressure, a positive breath holding test (53 subjects) detected latent arterial hypertension in 34 subjects (positive predictive value - 64 %). Conclusions.
