Материалы и методы. В исследование включено 57 пациентов (29 мужчин, средний возраст 50,2 ± 13,7 года),  с АГ 1-2-й степени, подтвержденной результатами суточного мониторирования АД (СМАД)  без поражения органов-мишеней и ассоциированных клинических состояний при рутинном обследовании. До начала и через 8 недель назначения стартовой антигипертензивной терапии выполняли СМАД, неинвазивный анализ центральной пульсовой волны и измерение скорости распространения пульсовой волны (СРПВ) с использованием системы SphygmoCor (AtCor Medical, Австралия). Исходные показатели центральной пульсовой волны и СМАД сопоставляли в подгруппах, выделенных на основании клинического измерения АД и СМАД через 8 недель лечения: 1) отсутствие ответа - клиническое АД ≥ 140/90 мм рт. ст., АД-24ч ≥ 125/80 мм рт. ст.; 2) истинный ответ - клиническое АД &lt; 140/90 мм рт. ст., АД-24ч &lt; 125/80 мм рт. ст.; 3) скрытая неэффективность лечения -  клиническое АД &lt; 140/90 мм рт. ст., АД-24ч ≥ 125/80 мм рт. ст.; 4) скрытое отсутствие ответа (эффект белого халата), клиническое АД ≥ 140/90 мм рт. ст., АД-24ч &lt;125/80 мм рт. ст. Результаты.  Отсутствие ответа на стартовую терапию отмечено у 14 (24,6 %) пациентов; истинный ответ - у 21 (36,8 %); скрытая неэффективность лечения - у 11 (19,3 %); скрытое отсутствие ответа - у 11 (19,3 %).  Несмотря на то, что пациенты со скрытой неэффективностью лечения были достоверно моложе, чем больные с отсутствием ответа, эти две подгруппы имели сопоставимые исходные характеристики периферического и центрального АД, СРПВ. Заключение. У ранее не леченных больных с неосложненной АГ скрытая  неэффективность стартовой антигипертензивной терапии может быть ассоциирована с более высокой артериальной ригидностью.</p></abstract><trans-abstract xml:lang="en"><p>To compare baseline central pulse wave characteristics in relation to response to initial antihypertensive treatment in previously untreated hypertensive patients. Design and methods. The study included 57 subjects (mean age 50,2 ± 13,7 years) with untreated arterial hypertension grade 1-2 without target organ damage on routine examination. Antihypertensive therapy was initiated after baseline ambulatory blood pressure monitoring (ABPM) and pulse wave analysis. Response to treatment was classified by joint evaluation of ABPM and clinic BP after 8 weeks of treatment. The baseline pulse wave velocity (PWV) was compared in 4 groups: 1) true responders (TR, normal clinic and ambulatory BP), 2) masked responders (MR, elevated clinic and normal ambulatory BP), 3) masked non-responders (MNR, normal clinic and elevated ambulatory BP), 4) true non-responders (TNR, elevated clinic and elevated ambulatory BP). Results. There were 21 (36,8 %) TR, 11 (19,3 %) MR, 11 (19,3 %) MNR, 14 (24,6 %) TNR. MNR was the youngest group but their baseline central systolic, pulse BP and PWV were at least similar or even higher than in the other groups. Conclusion.
