<trans-abstract xml:lang="en"><p>The article reviews possible mechanisms responsible for drug resistance in hypertensive patients with obstructive sleep apnea. Data about more profound target organ damage, partly explained by activation of inflammation, endothelial dysfunction due to recurrent hypoxia episodes and by blood pressure pattern in apnea patients are presented. Stimulation of renin-angiotensin-aldosterone system has a negative impact on cardiovascular system and deteriorates sleep apnea severity. </p></trans-abstract><kwd-group xml:lang="ru"><kwd>синдром обструктивного апноэ во время сна</kwd><kwd>резистентная артериальная гипертензия</kwd><kwd>ожирение</kwd><kwd>воспаление</kwd><kwd>поражение органов-мишеней</kwd><kwd>альдостерон</kwd></kwd-group><kwd-group xml:lang="en"><kwd>obstructive sleep apnea syndrome</kwd><kwd>resistant hypertension</kwd><kwd>obesity</kwd><kwd>inflammation</kwd><kwd>target organ damage</kwd><kwd>aldosterone</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Sjostrom C., Lindberg E., Elmasry A., Hagg A., Svardsudd K., Janson C.
