В условиях развития персонифицированной медицины и фармакогеномики сохраняет свою актуальность поиск новых генетических факторов, влияющих на особенности клинических проявлений гипертонической болезни и ишемической болезни сердца (ИБС). Цель исследования — проанализировать взаимосвязь структурного полиморфизма A603G тканевого фактора с медиаторами иммунного повреждения эндотелия у больных гипертонической болезнью, ассоциированной с ишемической болезнью сердца. Материалы и методы. Генотип тканевого фактора определяли методом полимеразной цепной реакции у 105 больных гипертонической болезнью и ИБС, для определения уровня интерлейкина-8 (ИЛ-8), интерферона-гамма (ИФ-гамма), фактора некроза опухоли-альфа (ФНО-альфа) плазмы крови применяли иммуноферметный метод. Результаты. 24 пациента (13,3 %) были носителями АА генотипа, 48 больных — AG генотипа (45,7 %) и 33 больных (30,5 %) имели GG генотип. Средний возраст дебюта ИБС был достоверно ниже у больных с GG генотипом — 52,1 ± 2,5 года по сравнению с соответствующим показателем у больных с AA генотипом (59,2 ± 2,5 года) и AG генотипом (60,1 ± 1,8 года). У больных с GG генотипом уровень ИЛ-8 составил 9,81 ± 2,14 пкг/мл, ИФ-гамма — 1,58 ± 0,25 пкг/мл, что достоверно выше, чем у больных с АА генотипом (3,08 ± 0,59 и 0,99 ± 0,21 пкг/ мл соответственно, р &lt; 0,05). У всех больных ИБС уровень ФНО-альфа был выше референтных значений. Выводы. Таким образом, у больных гипертонической болезнью GG генотип тканевого фактора был ассоциирован с более ранним дебютом ИБС и повышением уровня провоспалительных цитокинов ИЛ-8, ИФ-гамма, ФНО-альфа плазмы крови. </p></abstract><trans-abstract xml:lang="en"><p>Background. Genetic factors have clinical significance in arterial hypertension and coronary heart disease (CHD). Objective. To assess A603G tissue factor gene polymorphism and mediators of immune inflammation in patients with arterial hypertension and СHD. Design and methods. The A603G genotype was determined in a sample of 105 patients with arterial hypertension and CHD by a polymerase chain reaction-restriction length polymorphism (PCR-RFLP)-based method. We studied the mediators of immune inflammation — interleukin-8 (IL-8), interferon-gamma (IF-gamma), tumor necrosis factor-alpha (TNF) and interleukin-10 (IL-10). Results. 24 patients (13,3 %) were the carriers of AA genotype, patients had AG and GG genotype. CHD was diagnosed at the age of 52,1 ± 2,48 years in patients with GG genotype compared to 59,2 ± 2,54 years in AA genotype group and 60,1 ± 1,84 years in AG genotype group (p &lt; 0,05) . Levels of IL-8 (9,81 ± 2,14 pkg/ml) and IF-gamma (1,58 ± 0,25 pkg/ml) were elevated in patients with GG genotype in comparison with AA genotype patients (3,08 ± 0,59 и 0,99 ± 0,21 pkg/ml, respectively, p &lt; 0,05). Conclusion.
