Артериальная жесткость является перспективным фактором оценки риска развития сердечно-сосудистых осложнений и поражения органов-мишеней у пожилых.</p><p>Цель исследования — оценить степень жесткости сосудистой стенки и скорость ее роста у лиц старше 60 лет, используя сердечнолодыжечный сосудистый индекс (СЛСИ), а также выявить ассоциации значения СЛСИ с наличием сердечно-сосудистых осложнений.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Одномоментное поперечное исследование случайной выборки (n = 200) людей в возрасте 60+ в Башкортостане и Санкт-Петербурге. Методы обследования включали: измерение СЛСИ с помощью компьютерного комплекса объемной сфигмографии Vasera VS-1500, антропометрию, опрос и изучение амбулаторных карт, оценку медикаментозного лечения, лабораторные тесты (уровень глюкозы крови, липидограмма).</p></sec><sec><title>Результаты</title><p>Результаты. Величина СЛСИ зависит от возраста и носит нелинейный характер. Формула для расчета прогнозируемой величины СЛСИ у людей в возрасте 60+ с сердечно-сосудистыми заболеваниями (ССЗ) в анамнезе была получена при обследовании лиц в Башкортостане: среднее (Ср) ± стандартное отклонение (СО) (жен) = (7,5454 + 0,01426 × возраст + 0,000002445 × возраст3) ± (-11,8225 + 0,3681 × возраст - 0,002621 × возраст2); Ср ± СО (муж) = (12,1277 - 0,06924 × возраст + 0,000006745 × возраст3) ± (12,5238 - 0,2833 × возраст + 0,001730 × возраст2). Полученная формула была валидизирована в Санкт-Петербурге. Величина СЛСИ выше прогнозируемых значений была ассоциирована с высоким риском выявления острого нарушения мозгового кровообращения (ОНМК) в анамнезе. Увеличение СЛСИ на 1 единицу повышало вероятность ОНМК в анамнезе в 1,5 раза независимо от пола, возраста, сопутствующих заболеваний и региона проживания ([отношение шансов (95-процентный доверительный интервал) = 1,66 (1,16-2,38)]).</p></sec><sec><title>Выводы</title><p>Выводы. По результатам исследования была определена скорость прироста СЛСИ в зависимости от пола и возраста, и выведена формула его расчета у пожилых людей c ССЗ. Определены значения СЛСИ, ассоциированные с перенесенным ОНМК у людей в возрасте 60+.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Arterial stiffness is an independent prognostic factor for cardiovascular events and target organ damage related to hypertension.</p></sec><sec><title>Objective</title><p>Objective. To determine the rate of age-related increase of cardio-ankle vascular index (CAVI) and to investigate the association between CAVI and cardiovascular events in older adults.</p></sec><sec><title>Design and methods</title><p>Design and methods. This cross-sectional study was conducted on 200 older adults aged 60+ in Bashkortostan and St Petersburg. The following parameters were assessed: arterial stiffness (CAVI assessed by Vascular Screening System VaSera VS-1500), anthropometry, medical history and laboratory tests (glucose, lipids).</p></sec><sec><title>Results</title><p>Results. The CAVI is nonlinearly associated with age. In Bashkortostan, it was determined using the following equation: Mean ± SD (women) = (7,5454 + 0,01426 × age + 0,000002445 × age3) ± (-11,8225 + 0,3681 x age — 0,002621 × age2); Mean ± SD (men) = (12,1277-0,06924 × age + 0,000006745 × age3) ± (12,52380,2833 × age + 0,001730 × age2). The equation was validated in St Petersburg. CAVI was a higher predicting score in patients with a history of stroke than in those without a history of stroke. An increase in CAVI by 1 unit was associated with the 1,5-fold higher probability of a stroke in history, even after adjusting for age, gender and multimorbidity [odds ratio (95 % confidence interval) = 1,66 (1,16-2.38)].</p></sec><sec><title>Conclusion</title><p>Conclusion. We determined the rate of CAVI score increase with age. We developed and validated a new equation to predict CAVI in older adults in different age.
