Легочная артериальная гипертензия (ЛАГ) — это группа заболеваний, характеризующаяся поражением внутрилегочных сосудов и развитием правожелудочковой недостаточности. Патогенез ЛАГ до конца не изучен. К одному из звеньев патогенеза относят ремоделирование экстрацеллюлярного матрикса. Однако большое разнообразие структур и белков, входящих в его состав, а также сложность взаимосвязей между ними значимо затрудняет изучение и представляет собой большое поле для исследовательской работы.</p><p>Цель исследования — изучение особенностей экспрессии и распределения белков экстрацеллюлярного матрикса у пациентов без ЛАГ и с ЛАГ.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В работе использовался архивный аутопсийный материал легких от 16 пациентов с различными типами ЛАГ, в качестве группы сравнения был отобран аутопсийный материал от 7 пациентов, умерших от гемобластозов. Помимо гистологического исследования, проводилось иммуногистохимическое исследование (ИГХИ) со следующими маркерами: матриксная металлопротеиназа 9 типа (ММР9), filamin A, fibulin 5, collagen V, surfactant protein A, белок S 100 (S 100), Ki67.</p></sec><sec><title>Результаты</title><p>Результаты. Все пациенты с ЛАГ имели продвинутые морфологические стадии легочной гипертензии с формированием плексиформных структур. При ИГХИ легких пациентов с ЛАГ было показано повышение экспрессии ММР9 и тенденция к увеличению числа нервных волокон по сравнению с группой пациентов без ЛАГ, тогда как экспрессия filamin A, fibulin 5, collagen V, surfactant protein А в легочных артериях и паренхиме была снижена. Следует отметить, что, несмотря на разнородность выборки пациентов, было показано однотипное изменение профиля экспрессии белков экстрацеллюлярного матрикса в легочных артериях и паренхиме легких, что требует дальнейшего углубленного изучения на большей выборке пациентов.</p></sec><sec><title>Выводы</title><p>Выводы. Изучение маркеров экстрацеллюлярного матрикса и особенностей иннервации легких при ЛАГ у людей будет способствовать поиску таргетных препаратов и привлечению интервенционных технологий для лечения заболевания.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Pulmonary arterial hypertension (PAH) is a group of diseases characterized by the damage to the intra pulmonary vessels and the development of right ventricular insufficiency. The pathogenesis of PAH is not fully understood. Remodeling of the extracellular matrix is one suggested mechanism. However, the wide variety of its structures and proteins, as well as the complexity of the relationships between them represent a large field for research work. The aim of the study was to study the features of expression and distribution of extracellular matrix proteins in patients without and with PAH.</p></sec><sec><title>Design and methods</title><p>Design and methods. Archival autopsy material of lungs from 16 patients with various types of PAH was used, autopsy material from 7 patients who died from hemoblastosis was selected as a comparison group. In addition to histological examination, immunohistochemical examination was performed with the following markers: matrix metalloproteinase 9 (MMP9), filamin A, fibulin 5, collagen V, surfactant protein A, SI00, Ki67. </p></sec><sec><title>Results</title><p>Results. All patients with PAH had advanced morphological stages of pulmonary hypertension with the formation of plexiform structures. Immunohistochemical examination of the lungs of patients with PAH showed an increase in MMP9 expression and a tendency to increase the number of nerve fibers compared to the group of patients without PAH, while the expression of filamin A, fibulin 5, collagen V, surfactant protein A in the pulmonary arteries and parenchyma was reduced. It should be noted that despite the heterogeneity of the patient’s sample, a similar change in the expression profile of extracellular matrix proteins in the pulmonary arteries and lung parenchyma was shown, which requires further in-depth study in a larger sample of patients. </p></sec><sec><title>Conclusions</title><p>Conclusions.
