Сохраняющиеся высокие уровни заболеваемости и смертности трудоспособного населения от кардиоваскулярных заболеваний в России требуют поиска новых целей для скрининговых программ и последующей разработки научно обоснованных моделей профилактики в организованных коллективах, показывающих свою клинико-экономическую эффективность при долгосрочных проспективных наблюдениях.</p><p>Цель исследования — провести комплексную оценку прогностических факторов развития артериальной гипертензии (АГ) по данным 8-летнего проспективного исследования организованной выборки с учетом половых и гендерных особенностей.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведено двухэтапное исследование: одномоментное сплошное исследование организованной популяции (400 человек) с последующим 8-летним проспективным наблюдением группы относительно здоровых лиц без клинических проявлений сердечно-сосудистых заболеваний (ССЗ) (193 человека, в возрасте 21–64 года, средний возраст 49,0 [42,0; 58,0], из них мужчин 48,2 %). Изучаемые показатели I этапа: социодемографические характеристики; клинические и физикальные показатели; поведенческие факторы; семейный анамнез ранних ССЗ; профессиональное интервью и степень напряженности труда; психологические характеристики; биохимические маркеры стресса; показатели кардиоваскулярного ремоделирования. Изучаемые показатели II этапа: впервые установленные случаи АГ.</p></sec><sec><title>Результаты</title><p>Результаты. Структура прогностических факторов АГ зависела от пола. При этом среди женщин прогностическими факторами 8-летней заболеваемости АГ явились: семейный статус (вдова) (отношение рисков (ОР) 10,1), уровень диастолического артериального давления (АД) (ОР 1,1) и физической активности вне работы (ОР 0,3). У мужчин кроме поведенческих (стаж курения, ОР 1,1) и клинических (высокое нормальное АД, ОР 4,1) факторов также наблюдался независимый вклад психологического фактора (личностная тревожность, ОР 6,5).</p></sec><sec><title>Заключение</title><p>Заключение. Полученные нами данные обосновывают необходимость полипрофессионального подхода (врач-интернист — психиатр — медицинский психолог) к профилактике ССЗ в изученной организованной популяции с учетом полоролевых особенностей.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Persistent high levels of morbidity and mortality of the working-age population from cardiovascular diseases (CVD) in Russia require the search for new targets for screening programs and subsequent development of evidence-based prevention models in organized populations showing their clinical and economic effectiveness in long-term prospective follow-up.</p></sec><sec><title>Objective</title><p>Objective. To carry out a comprehensive assessment of the prognostic factors of arterial hypertension (AH) according to the data of 8-year prospective study of an organized sample taking into account gender and sex characteristics.</p></sec><sec><title>Design and methods</title><p>Design and methods. We conducted a two-stage study: a one-stage integral study of an organized population (400 people) followed by a prospective 8-year follow-up of a group of relatively healthy subjects without clinical manifestations of CVD (193 people aged 21 to 64 years, mean age 49,0 [42,0; 58,0], of whom men 48,2%). Stage I indicators under study: sociodemographic characteristics; clinical and physical findings; behavioral factors; family history of early CVD; occupational interview and degree of work stress; psychological characteristics; biochemical indices of stress response; cardiovascular remodeling indices. Stage II indicators under study: newly diagnosed cases of AH.</p></sec><sec><title>Results</title><p>Results. The pattern of prognostic factors for AH depended on gender. Among women, prognostic factors for 8-year incidence of AH were: marital status (widow) (Hazard ratio (HR) 10,1), diastolic blood pressure level (HR1,1), and physical activity outside work (HR0,3). In men, in addition to behavioral (smoking history, HR1,1) and clinical (high normal blood pressure, HR4,1) factors, there was also an independent contribution of a psychological factor (personality anxiety, HR6,5).</p></sec><sec><title>Conclusions</title><p>Conclusions.
