Однако у пациентов с хронической болезнью почек (ХБП) и аллергической реакцией на йод, несмотря на инфузионную и десенсибилизирующую терапию, очень высок риск развития осложнений после этой процедуры. Мы представляем клинический случай успешного и безопасного применения карбоксиангиографии при выполнении денервации почечных артерий у пациентки с резистентной артериальной гипертензией (АГ) в сочетании с сахарным диабетом 2-го типа и ХБП на фоне аллергической реакции на йодконтрастные вещества в виде анафилактоидной реакции в анамнезе (отек Квинке). Альтернативный вид ангиографии позволил нам провести успешное эндоваскулярное лечение АГ с достижением целевых уровней артериального давления в отсутствие осложнений вмешательства в ранние и отдаленные периоды наблюдений.</p></abstract><trans-abstract xml:lang="en"><p>Traditionally an iodine-containing contrast agent is used for renal arteries angiography. However, patients with chronic kidney disease (CKD) and allergic reaction to iodine, despite infusion and desensitization therapy, have a very high risk of developing complications after procedure. We present a clinical case of successful and safe use of carboxyangiography for renal artery denervation in a patient with resistant arterial hypertension (HTN) in combination with type 2 diabetes mellitus and CKD against the background of an allergic reaction to iodine contrast agents in the form of an anaphylactoid reaction in anamnesis (angioedema).
