В рамках эпидемиологического исследования ЭССЕ-РФ были обследованы 1592 жителя Санкт-Петербурга (мужчин — 567, женщин — 1025). Антропометрия, измерение артериального давления (АД), биохимический анализ крови выполнены согласно стандартным процедурам. В ходе инструментального обследования определялись: толщина комплекса «интима-медиа» (ТИМ) сонных артерий (на аппарате «My Sono U6»); каротидно-феморальная скорость распространения пульсовой волны (кфСРПВ), сердечно-лодыжечный сосудистый индекс (CAVI), лодыжечно-плечевой индекс (ЛПИ) на аппарате «VaSera VS‑1500». Оценка сердечно-сосудистого риска определена по шкале SCORE. </p></sec><sec><title>Результаты</title><p>Результаты. У 955 (76,7 %) человек не найдено никаких признаков поражения артерий. У пациентов низкого и промежуточного риска по шкале SCORE выявлена наибольшая распространенность субклинического поражения сосудов. Чаще всего встречалось утолщение ТИМ (212 человек, 24,7 %), а наиболее редко — снижение ЛПИ (18 человек, 2,1 %). Вне зависимости от пола ТИМ коррелирует с антропометрическими показателями: окружностью талии и индексом массы тела. У женщин выявлена ассоциация также с уровнем систолического АД, а у мужчин были зарегистрированы более высокие значения кфСРПВ‑В и CAVI по сравнению с женщинами. Доля лиц с ТИМ &gt; 0,9 мм была также выше среди мужчин. КфСРПВ‑В и ТИМ были ассоциированы между собой (β = 0,43, p &lt; 0,0001).</p></sec><sec><title>Заключение</title><p>Заключение. В российской популяции отмечается невысокая распространенность субклинического поражения артерий по данным различных методов диагностики. Проведение оценки субклинического поражения сосудов наиболее целесообразно у лиц с низким и промежуточным риском; уровень риска SCORE коррелировал со значениями CAVI, ЛПИ и ТИМ. Распространенность повышения индекса CAVI и ТИМ значимо выше среди мужчин, у которых чаще встречается артериальная гипертензия, гипертриглицеридемия и гипергликемия. Изолированное повышение CAVI вне зависимости от возраста ассоциировано с более высоким уровнем АД, большей распространенностью артериальной гипертензии и более старшим возрастом, а у женщин — также с гипертриглицеридемией и гипергликемией.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective. To estimate the prevalence of subclinical organ damage by basic existing screening methods, and correlation with traditional risk factors in a sample of St Petersburg inhabitants.</p></sec><sec><title>Design and methods</title><p>Design and methods. The study was carried out as a part of ESSE-RF study. Totally, 1592 St Petersburg inhabitants (567 men and 1025 women) were examined. Anthropometry, blood pressure measurement and biochemistry were performed according to standard protocols. During instrumental investigation intima-media thickness (IMT) by My Sono U6; carotid-femoral pulse wave velocity (cfPWV), cardio-ankle vascular index (CAVI) and ankle brachial index (ABI) by VaSera VS‑1500 were assessed. Сardiovascular risk was assessed by SCORE-scale. </p></sec><sec><title>Results</title><p>Results. There is no subclinical vascular damage found in the majority of participants (n = 955, 76,7 %). The highest prevalence of subclinical organ damage is detected in patients with low and intermediate SCORE risk. Increasing of IMT more than 0,9 mm is the most prevalent marker (n = 212, 24,7 %) and ABI lower than 0,9 (n = 18, 2,1 %) was the less prevalent marker comparing with other methods. IMT correlates with waist circumference and BMI; besides this, a correlation with systolic BP is found only in women. Cardiovascular risk score correlated with CAVI, ABI and IMT. СfPWV and CAVI mean values, prevalence of IMT &gt; 0,9 mm are significantly higher in males comparing with females. Only association between cfPWV and IMT is significant (β = 0,43, p &lt; 0,0001).</p></sec><sec><title>Conclusion</title><p>Conclusion. Low prevalence of subclinical organ damage is revealed according to different diagnostic methods. Evaluation of subclinical organ damage is reasonable for patients with low and intermediate risk; the risk level according to SCORE correlates with CAVI, ABI and IMT data. The prevalence of high CAVI and IMT is higher among men with hypertension, hypertriglyceridemia and hyperglycemia.
