Материалы и методы. Был проведен ретроспективный анализ историй болезни всех пациентов с первичными ВМК, поступивших в первичное сосудистое отделение Дюртюлинской центральной районной больницы Республики Башкортостан с 2015 по 2021 год. Регистрировались клинико-демографические данные и результаты стандартных лабораторно-инструментальных обследований, а также функциональные исходы через 90 суток по модифицированной шкале Рэнкина (mRs). Результаты. Были проанализированы данные 120 пациентов. Значимыми предикторами неблагоприятного функционального исхода (mRs 3 балла и более) первичного ВМК через 90 дней оказались: более высокий уровень натрия (р = 0,014) в крови, тяжесть инсульта по шкале NIHSS (р = 0,0001) и большая длительность интервала QT (р = 0,05). Заключение. Таким образом, ранними предикторами неблагоприятного функционального исхода первичного ВМК через 90 суток, наряду с исходной тяжестью неврологического дефицита, были уровень электролитов в крови, а также признаки нарушения реполяризации миокарда. Полученные данные позволят выделить на этапе поступления в стационар группу риска по развитию неблагоприятного функционального исхода, а также оптимизировать ведение пациентов с первичными ВМК за счет ранней коррекции выявленных факторов.</p></abstract><trans-abstract xml:lang="en"><p>Objective. The aim of the study was to identify clinical, demographic, laboratory and instrumental predictors of the functional outcome of primary intracerebral hemorrhage (ICH) after 90 days among the parameters available within the first hour after the patient's admission to the hospital. Design and methods. A retrospective analysis of the medical records of all patients with primary ICH admitted to the primary vascular department of the Dyurtyulinskaya Central District Hospital of the Republic of Bashkortostan from 2015 to 2021 was performed. Clinical, demographic data and the results of standard laboratory and instrumental examinations were recorded, as well as functional outcomes after 90 days according to the modified Rankin scale (mRs). Results. Data from 120 patients were analyzed. The following predictors of unfavorable functional outcome (mRs 3 points or more) of primary ICH after 90 days were identified: high blood sodium level (p = 0,014), stroke severity according to the NIHSS scale (p = 0,0001), longer QT interval duration (p = 0,05). Conclusion. Thus, early predictors of unfavorable functional outcome of primary ICH after 90 days, along with the initial severity of neurological deficit, were electrolytes level, as well as signs of myocardial repolarization disorders.
