Метаболический синдром (МС) и фибрилляция предсердий (ФП) — взаимосвязанные патологии, в патогенезе которых важную роль играет окислительный стресс. Однако комплексная оценка системы перекисного окисления липидов–антиоксидантной защиты (ПОЛ-АОЗ) в плазме и эритроцитах при их коморбидности недостаточно изучена. Цель исследования — сравнительное изучение особенностей системы ПОЛ-АОЗ в плазме и эритроцитах у пациентов с МС и ФП относительно больных с МС (без ФП), ФП (без МС) и здоровых лиц. Материалы и методы. В одномоментном наблюдательном исследовании типа «случай–контроль» участвовали 237 человек в возрасте 55–70 лет, разделенные на 4 группы: МС без ФП (n = 50), МС с ФП (n = 89), ФП без МС (n = 48) и группа сравнения (условно здоровые лица) (n = 50). В плазме и эритроцитах определяли концентрацию тиобарбитуровой кислоты–реактивных продуктов (ТБК-РП, отражающих содержание малонового диальдегида), активность супероксиддисмутазы (СОД) и каталазы (КАТ) спектрофотометрическими методами. Результаты. В эритроцитах всех групп пациентов активность СОД и КАТ была значимо ниже, чем в группе сравнения (p &lt; 0,001), без различий между группами пациентов. Уровень ТБК-РП в эритроцитах был ниже в группе ФП без МС. В плазме уровень ТБК-РП был значимо выше, а активность СОД и КАТ — ниже во всех группах пациентов по сравнению с группой условно здоровых участников. Наиболее выраженное повышение ТБ-КРП в плазме отмечено при коморбидности МС и ФП. Заключение. При коморбидности МС и ФП наблюдается дисбаланс в системе ПОЛ-АОЗ с преобладанием внеклеточной липопероксидации (повышение ТБК-РП в плазме) и угнетением антиоксидантной защиты в эритроцитах и плазме. Выявленные изменения, особенно максимальное повышение ТБК-РП и снижение КАТ в плазме, могут служить основанием для рассмотрения антиоксидантной терапии.</p></abstract><trans-abstract xml:lang="en"><p>Background. Metabolic syndrome (MetS) and atrial fibrillation (AF) are interrelated pathologies, oxidative stress plays important role in their pathogenesis. However, a comprehensive assessment of the lipid peroxidationantioxidant defense (LPO-AOD) system in both plasma and erythrocytes in comorbid MetS and AF remains insufficiently studied. Objective. To conduct a comparative study of the features of the LPO-AOD system in plasma and erythrocytes in patients with both MetS and AF, relative to patients with MetS (without AF), AF (without MetS), and healthy individuals. Design and methods. A cross-sectional observational case-control study included 237 individuals aged 55–70 years, divided into 4 groups: MetS without AF (n = 50), MetS with AF (n = 89), AF without MetS (n = 48), and a comparison group (apparently healthy individuals, n = 50). The concentration of TBARS and the activity of superoxide dismutase (SOD) and catalase (CAT) were determined in plasma and erythrocytes using spectrophotometric methods according to the validated modifications. Results. In all patient groups, SOD and CAT activity in erythrocytes was significantly lower than in the comparison group (p &lt; 0,001), with no significant differences between the patient groups. The TBARS level in erythrocytes was lower in the AF without MetS group. In all patient groups, the TBARS plasma level was significantly higher, while SOD and CAT activity was lower than in the comparison group. The most pronounced increase in plasma TBARS was noted in the group with comorbid MetS and AF. Conclusion. The comorbid MetS and AF are characterized by an imbalance in the LPO-AOD system, manifesting as a predominance of extracellular lipoperoxidation (elevated plasma TBARS) and suppression of antioxidant defense in both erythrocytes and plasma.
