В статье обсуждаются четыре положения, которые содержатся в согласованном мнении российских экспертов по ведению артериальной гипертензии у пациентов 80 лет и старше, а также у пациентов со старческой астенией. Эти положения касаются рекомендаций по пороговым и целевым уровням артериального давления, по выбору стартовой терапии (монотерапии или комбинированной терапии) и доз (высоких или низких) препаратов при инициации лечения у пациентов 80 лет и старше без признаков старческой астении и при наличии таких симптомов. По мнению авторов, данные положения требуют некоторой коррекции, так как не полностью соответствуют результатам исследования HYVET. Вызывают сомнения и споры уровень доказанности ряда положений и класс рекомендаций. В представленной статье авторы приводят сопоставление с Европейскими рекомендациями 2013 года, в качестве обоснования дается анализ крупных рандомизированных клинических исследований и метаанализов. 

The article discusses four statements of the position agreement of the Russian experts on the treatment of arterial hypertension in patients over 80 years old, as well as in patients with senile asthenia. These statements consider recommendations on the threshold and target blood pressure levels, on the initial antihypertensive therapy (monotherapy or combination therapy), the initial doses of the drugs (low or high) in patients over 80 years old with or without senile asthenia. The authors believe that these statements require some correction as they do not fully correspond to the results of the HYVET trial. The level of evidence and recommendation class stated in the position agreement are disputable and give rise to doubt. The authors compare the position agreement by the Russian experts and European guidelines 2013 and present the evidence of the large randomized clinical trials and meta-analyses.
