Гипертоническая болезнь (ГБ) представляет собой наиболее значимый фактор, ассоциированный с развитием хронической сердечной недостаточности (ХСН) с сохраненной фракцией выброса левого желудочка (ФВ ЛЖ). Системное низкоинтенсивное воспаление считается важным элементом патогенеза и артериальной гипертензии, и ХСН. Цель исследования — с учетом гендерной принадлежности установить особенности показателей провоспалительного статуса у пациентов среднего возраста с ГБ, осложненной клинически нетяжелой ХСН с сохраненной ФВ ЛЖ. Материалы и методы. Обследовано 165 человек среднего возраста. Основную группу составили 104 пациента с ГБ, осложненной ХСН с сохраненной ФВ ЛЖ (в том числе 55 мужчин). Диагноз ХСН подтверждали повышенным содержанием в плазме крови N‑концевого предшественника мозгового натрийуретического пептида и данными теста с 6‑минутной ходьбой. У лиц основной группы была верифицирована 1‑я или 2‑я степень диастолической дисфункции левого желудочка (ДД ЛЖ). Группу сравнения составили 30 пациентов с ГБ II стадии (в том числе 15 мужчин), а группу контроля — 31 нормотензивное лицо (в том числе 15 мужчин). У всех обследованных определили содержание в сыворотке крови С‑реактивного белка (СРБ), фактора некроза опухоли альфа (ФНО-α), интерлейкина‑6 (ИЛ‑6). Полученные данные обработаны с помощью программы Statistica for Windows (версия 10.0). Результаты. В каждой группе обследованных лиц содержание маркеров провоспалительного статуса в крови оказалось нормальным. У мужчин основной группы уровни СРБ, ФНО-α, ИЛ‑6 значительно превышали таковые у женщин. У мужчин и женщин с ГБ в сочетании с ХСН с сохраненной ФВ ЛЖ величины каждого из этих показателей превосходили таковые у мужчин и женщин группы сравнения. Полученные данные у лиц основной группы были также оценены селективно с учетом особенностей ДД ЛЖ. Содержание СРБ, ФНО-α, ИЛ‑6 в крови у мужчин значительно превышало таковое у женщин (в пределах выборок с одинаковой степенью ДД ЛЖ). У пациентов со 2‑й степенью ДД ЛЖ наблюдались более высокие уровни СРБ, ФНО-α, ИЛ‑6 в крови, чем у субъектов с 1‑й степенью ДД ЛЖ (в пределах мужской и женской выборок). Заключение. По сравнению с женщинами 23(5) / 2017 мужчинам с ГБ, осложненной ХСН с сохраненной ФВ ЛЖ, свойственно значительно более высокое содержание в крови СРБ, ФНО-α, ИЛ‑6. Присоединение к гипертензии ХСН с сохраненной ФВ ЛЖ сопровождается усилением напряженности провоспалительного паттерна как среди мужчин, так и среди женщин. Принадлежность ко 2‑й степени ДД ЛЖ ассоциирована с ухудшением провоспалительного паттерна относительно 1‑й степени ДД ЛЖ.</p></abstract><trans-abstract xml:lang="en"><p>Background. Hypertension (HTN) represents the most significant factor associated with the development of chronic heart failure (CHF) with preserved left ventricular ejection fraction (LVEF). Low-intensity systemic inflammation is considered an important element for the pathogenesis of both HTN and CHF. Objective. To set gender-dependent features of pro-inflammatory status in middle-aged patients with HTN complicated by non-severe CHF with preserved LVEF. Design and methods. The study involved 165 middle-aged subjects. The main study group comprised 104 patients (including 55 males) with HTN complicated by CHF with preserved LVEF. CHF diagnosis was confirmed by the elevated blood plasma N‑terminal brain natriuretic peptide precursor and 6‑minute walk test. Also, the main group was stratified depending on the presence of the 1st or the 2nd grade of left ventricular diastolic dysfunction (LVDD). The comparison group consisted of 30 patients with stage II HTN (including 15 males), and a control group — of 31 normotensive individuals (including 15 males). In all subjects we measured serum levels of C‑reactive protein (CRP), tumor necrosis factor alpha (TNF-α), interleukin‑6 (IL‑6). The data were processed by the program Statistica for Windows (version 10.0). Results. In each group the blood markers of proinflammatory status were normal. Males of the main group showed higher levels of CRP, TNF-alpha, IL‑6 than females. Hypertensive males and females with CHF with preserved LVEF demonstrated significantly higher indices of all parameters than in the comparison group. We also analyzed data depending significantly greater than in subjects with the 1st LVDD grade (both among males and females). Conclusions. In comparison with females, males with HTN complicated by CHF with preserved LVEF have significantly higher CRP, TNF-α, IL‑6 serum levels. Development of CHF with preserved LVEF in HTN patients is associated with increased intensity of the pro-inflammatory pattern among both males and females.
