Сосудодвигательная функция эндотелия у больных ишемической болезнью сердца с желудочковыми нарушениями ритма, возникающими на фоне миокардиальной ишемии

Аннотация

Оценить состояние сосудодвигательной функции эндотелия у больных ИБС с желудочковыми нарушениями ритма (ЖНР), возникающими на фоне ишемии.
Материал и методы: Обследовано 48 больных ИБС без выраженной артериальной гипертензии и симптомов хронической сердечной недостаточности (фракция выброса > 50 %) с электрокардиографическими признаками ишемии во время пробы с физической нагрузкой (ФИ) на велоэргометре, которые были разделены на 2 равные группы по признаку появления (I группа) или отсутствия (II группа) воспроизводимых ЖНР на пике ФН или в восстановительном периоде (ВП). Также были выполнены пробы с реактивной гиперемией плечевой артерии для оценки эндотелий-зависимой вазодилатации (ЭЗВД) и с нитроглицерином для оценки эндотелий-независимой вазодилатации (ЭНВД) с использованием ультразвука высокого разрешения и определение концентрации эндотелина-1 (ЭТ-1) в плазме методом иммуноферментного анализа.
Результаты: В I группе больных ЭЗВД плечевой артерии оказалась достоверно меньше, а длительность ишемических изменений ЭКГ в ВП больше, чем во II группе. Не было выявлено достоверных различий в величинах плазменной концентрации ЭТ-1 между группами.
Выводы: У больных ИБС с ЖНР, возникающими на фоне ишемии, сосудодвигательная функция эндотелия нарушена в большей степени, а ишемия в пробе с ФН сохраняется дольше, чем у больных ИБС без ЖНР
